Children birth to two years old are served at East Leesville, Juz us & North Polk

Documents Needed:

• Child’s Official Birth Certificate
• Child’s Social Security Card
• Child’s Shot Record
• Family’s Proof of Income (most recent 2 consecutive pay stubs or LES statement (military))
• Proof of residency- FOUR YEAR OLDS ONLY- This is for transportation purposes.

Applications are taken Monday- Thursday (8:30-11:30 AM)
